Model Withdrawal Form

This is the statutory model withdrawal form, Annex I part B to Directive 2011/83/EU. Use it only if you want to withdraw from the contract — filling it in is not mandatory, but we must make it available.

Faster: the withdrawal function

You do not have to use this form. Our withdrawal function (currently only available in Dutch) does the same thing and is faster: you get a confirmation immediately, you can choose per item what you are returning, and you do not need to print or scan anything. This model withdrawal form remains available alongside it, not instead of it — the law requires it to be available, regardless of which other route you choose.

How to send it

Print this form, fill it in and send it to service@dreamglow.nl or to our address above. If you fill it in by email, you do not need to sign anything — the signature line is only for a form you send on paper.

As soon as we receive your withdrawal, we confirm this by email on a durable medium. What happens next — returning the products, refunding you — is set out in the terms and conditions, articles 10 to 12.
